Providence Health & Services Behavioral Health Psychiatric Outreach Coordinator in Missoula, Montana
Providence is calling a Social Worker (Mental Health, Day Shift, 0.5 FTE/20 hours per week) to Providence Medical Group in Missoula, Montana. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Provide coordinated community outreach, assessment and therapy/ counseling services for patients within the medical homes, other health clinics, and other locations.
Coordinate and participate in the clinical service delivery, education and community relations activities, and in collaboration with other aspects of the Population Health Management and Accountable Care Organization Initiatives.
Develop strategies that will result in meeting the patient’s needs, maximizing patient satisfaction, supporting a positive community image for the organization, and assists the facility in meeting its operational goals
Required qualifications for this position include:
Master’s degree in Social Work, Psychology or related field
Ability to become Certified as a Mental Health Professional within 90 days of hire
Minimum of three to five years of experience in the behavioral health field
Ability to cover after hours on-call responsibilities, work under pressure, and meet deadlines
Preferred qualifications for this position include:
- State Licensure as a Professional Social Worker/Counselor
