Providence Population Health Resource Specialist *Temporary* in Missoula, Montana
Providence is calling a Population Health Resource Specialist (Pmg Clinical Support Services, Days, 1.0 FTE) to St. Patrick Hospital in Missoula, Montana. In this position you will:
Work collaboratively with all providers and staff members, with a focus and emphasis on nursing and medical assistant staff members, to develop clinical support processes that ensure the delivery of efficient, compassionate, and high quality care.
Work within a centralized team, helping providers, clinic staff and management to effectively, efficiently and accurately fulfill the duties and responsibilities of the centralized services department in support of our Patient Centered Medical Home objectives.
Duty include chronic disease management, preventative health coordination of services, external result entry, prior authorization of medication and procedures, referral coordination, tracking and managing the referral work queue.
Responsible for obtaining pertinent patient information for referrals, prior authorizations or updating patient medical records.
Responsible for follow up on the patients’ appointment and notes, and assuring it is received by the referring provider.
Coordinate communication with nurses, providers and other care team members.
Function as a member/leader of Interdisciplinary care delivery team.
Take emergency action when required.
Help patient navigate health care system.
Responsible for follow-up of referrals to specialists, other providers, and other agencies.
Utilize available resources effectively by appropriate consultation and collaboration to maximize effectiveness of care.
Document assessments, interventions, and evaluation of care accurately, objectively, and timely.
Provide clear, concise pertinent patient reports/information.
Required qualifications for this position include:
- H.S. Diploma or GED
Preferred qualifications for this position include:
Current Licensed Practical Nurse (LPN) licensure
One (1) year referral tracking and coordination experience.
