Providence Health & Services RN Case Manager - Utilization Review in Missoula, Montana
Providence is calling a RN Case Manager (Utilization Review, Day Shift, 1.0 FTE) to Providence St. Patrick Hospital in Missoula, Montana. In this position you will:
Address variance findings through research, planning and system improvement using IDT approach incorporating LEAN/established change methodologies. Participates in identified LEAN/change activities.
Collaborate with UR/UM to track potential savings related to negotiated service arrangements.
Continuously assess, develop, and implement discharge patient/family needs and work with appropriate healthcare provider to procure necessary services.
Coordinate finalization of all discharge related recommendations.
Demonstrate caring and involved behaviors to patient/family members and play an advocacy role to ensure positive outcomes.
Develop and direct continuum of care map in collaboration with interdisciplinary team.
Document avoidable days/delays with attributions as evidenced by Meditech audit reports on quarterly basis.
Required qualifications for this position include:
Graduate of an accredited school of nursing.
Current licensure as a Registered Nurse in the State of Montana.
One year of clinical nursing experience.
Preferred qualifications for this position include:
Current Case Manager Certification preferred.
Three years of clinical nursing experience.
Hospital case management experience.
