Providence is calling a Social Worker (Social Services,Variable Shift, 0.0 FTE) to St. Patrick Hospital in Missoula, Montana. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
In this position you will:
Assess patient/family learning needs, abilities, preferences and readiness to learn. Complete on going assessments and documents according to policy. Communicate with other members of the health care team regarding the patient’s needs, plan and response to care. Evaluate response to plan and makes adjustments as indicated.
Assess, collaborate and coordinate patient’s discharge planning needs and readiness for discharge with other members of the care team.
Consult regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.
For the ages listed, screen, assess, monitor, document and interpret psychosocial, spiritual, environmental, self-care, educational, and discharge planning needs.
In collaboration with the patient/family, and in a developmentally and culturally appropriate manner, implement the plan of care according to established Social Work policy.
Maintain current knowledge of reimbursement and regulatory issues of the payors most frequently seen with the patient population.
Obtain a history through the interview process, maintain ongoing monitoring/assessment of assigned patients.
Participate in the development of patient plan of care to include patient problems, goals, evaluations and interventions.
Report symptoms and significant changes in patient’s condition to appropriate personnel.
Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met. Provides community resource information to patient/family and evaluates the effectiveness of teaching, making adjustments as necessary. Documents education process on appropriate screen/format.
Work with health care team and outside resources to facilitate optimum patient care delivery.
Educate health team colleagues about Social Work. Including the role and unique needs of the case-managed patient populations and serves as a resource for physicians and other departments.
Required qualification for this position include:
- Master’s degree in Social Work
Preferred qualification for this position include:
- 2 years experience in medical social work
